Collaborative Care Psychiatrist Position

The psychiatric consultant plays a vital role in supporting the collaborative care team, which includes the primary care provider, patient, and behavioral health care manager, through consistent consultation on a designated patient caseload. This role involves advising the primary care provider on suitable medications and treatment approaches while also offering educational support to the care team regarding the diagnosis and management of behavioral health issues.
Key Responsibilities:
1. Conduct regularly scheduled consultations (typically weekly) with behavioral health care managers to review cases. These consultations primarily focus on patients who are either new to the caseload or not showing expected improvement under their current treatment plan. Reviews can be performed via telephone, video, or in-person meetings.
2. Collaborate with behavioral health care managers to monitor the status and overall health of their assigned patient population. The psychiatric consultant utilizes a registry to track all patients and prioritize them for case reviews and treatment modifications. This registry can be managed through an EHR integrated with a spreadsheet or through a standalone clinical management system, which may or may not be linked to an EHR.
3. Propose treatment adjustments through the case review process or during ad hoc consultations, including medication recommendations and other interventions, within 24 hours of discussions with a behavioral health care manager or other team members.
4. Provide guidance on treatment for patients requiring more intensive or specialized mental health care, ensuring support in the medical setting until patients can transition to specialized care as needed.
5. Clearly communicate the limitations of case review consultations and treatment recommendations when the patient has not been evaluated in person. An example disclaimer for consultation notes in an EHR might read:
"The above treatment considerations and suggestions are based on consultation with the patient's behavioral health care manager and a review of information available in the registry. I have not personally examined the patient. All recommendations should be implemented with consideration of the patient's relevant prior history and current clinical status. Please feel free to reach out with any questions regarding the care of this patient."
6. Be available for occasional ad hoc telephone consultations with primary care providers for complex cases or challenging patient situations. Respond to calls from primary care providers and behavioral health care managers within one business day, and address urgent calls within one hour, if available. Coordinate with other psychiatric consultants for coverage during absences.
