Job Summary

We are seeking a skilled Home Health Registered Nurse (RN) to join our team. As a Home Health RN, you will play a vital role in supporting patient recovery and empowering them to reach their healthcare goals. You will work closely with our interdisciplinary team to provide clinical oversight and case management services to patients in their homes.

Key Responsibilities:

  • Provide comprehensive nursing care to patients in their homes, including assessments, treatment plans, and follow-up visits
  • Collaborate with other healthcare professionals to develop and implement patient care plans
  • Monitor patient progress and make adjustments to care plans as needed
  • Communicate effectively with patients, families, and caregivers to ensure seamless care transitions

Requirements

  • RN license in the state where you will be working
  • Two years of experience as a registered nurse, with home health experience preferred
  • Current driver's license
  • Current CPR and BCLS certification
