About the Role

Mountain Ridge Center in Franklin, NH is seeking a Part-Time Social Services Assistant to join our team. As a Social Services Assistant, you will provide administrative support to our social service staff, ensuring a seamless transition for patients and residents throughout their stay.

Key Responsibilities
  • Collaborate with patients/residents and their families to provide support and resource information.
  • Respond to issues identified by patients/residents and their families.
  • Facilitate patient/resident transfer within the nursing center to ensure a smooth transition and document accordingly.
  • Participate in the healthcare decision-making process within the nursing center under the direction of a qualified social worker and/or qualified consultant.
