Interim HealthCare of the Twin Cities is seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality home care services to patients in the Southwest Twin Cities Metro area.
Key Responsibilities:
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Conduct ongoing assessments of patients' conditions and develop and implement individualized care plans.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care.
- Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and progress.
- Document patient information accurately and timely, including progress notes and care plans.
Requirements:
- Current license as an LPN in the State of MN, active and in good standing.
- 2 years' experience as an LPN, with a strong background in home care and skilled nursing.
- Able to travel within the geographic area serviced by the office, with a valid driver's license, reliable transportation, and proof of auto insurance.
- CPR certification.
