Responsibilities:

* Provide patient counseling and referrals for patient needs
* Assist with coordinating patient care
* Maintain a customer service program to ensure high-quality service and customer satisfaction
* Participate in and initiate advancement of hospital, community, and patient relations
* Survey and follow-up with referring providers to ensure high-quality customer service
* Meet with patients to ensure customer satisfaction with the Program and hospital experience
* Utilize and/or create databases for tracking and following patients through outpatient clinical settings
* Organize and analyze program data on a regular basis
* Create and maintain referring physician informational databases

Requirements:

* Bachelor's Degree in Health Education, Social Work, Psychology, Nursing, or health-related field
* 12 months experience in a medical or clinical care setting
* Equivalency/Substitution: Will accept 48 months related experience in lieu of the Bachelor's Degree for a total of 60 months experience
* A license from the State of Oklahoma related to the required health field

Physical Requirements:

* Physical: Sitting for long periods of time
* Environmental: Office Environment

Departmental Preferences:

* Bilingual - English/Spanish
