We are seeking a highly skilled and dedicated Licensed Occupational Therapist to join our team at EmpowerMe Wellness. As a key member of our care coordination program, you will have the opportunity to practice at the top of your license and make a meaningful impact on the lives of seniors.

Key Responsibilities
  • Provide Occupational Therapy Services: Conduct assessments, treatment planning, and therapeutic interventions to support the functional well-being of older adults.
  • Support Care Coordination Efforts: Conduct regular wellness checkpoint visits, communicate with patients, providers, and caregivers, and assist patients with attending telehealth visits.
  • Maintain Professional Practices: Adhere to company-wide policies and clinical initiatives, ensuring the highest standards of care and professionalism.
  • Contribute to Program Development: Collaborate with the team to develop and implement programs that optimize patient outcomes and satisfaction.
Requirements
  • Education and Licensure: Completion of an accredited Occupational Therapy program and current state licensure in OT.
  • Communication and Interpersonal Skills: Excellent verbal and written communication skills, with the ability to work effectively with patients, providers, and caregivers.
  • Technical Skills: Proficiency in using technology to support care coordination and therapy services.
