About the Role
We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Almost Family - Marinette PD as a Home Care Specialist. As a Home Care Specialist, you will provide high-quality clinical services to patients in their homes, working under the direct supervision of a Registered Nurse and/or designated supervisor.
Key Responsibilities
- Provide Direct Patient Care: Deliver high-quality clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the healthcare team.
- Document Patient Visits: Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record utilizing the agency's electronic visit verification program.
- Participate in Care Coordination: Actively participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, other clinicians, agency management staff, and case coordinators.
- Ensure Confidentiality: Ensure the confidentiality of patient health information per agency policy.
Requirements
- Current LPN Licensure: Must have current LPN licensure in the state of practice.
- Driver's License and Vehicle: Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification: Current CPR certification required.
Preferred Qualifications
- Previous Home Care Experience: Previous home care experience preferred.
