Job DescriptionWe are seeking experienced Nurse Practitioners or Physician Assistants to join our team as Clinical Risk Assessors - Home Healthcare Specialists. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and providing personalized care recommendations.
- Collaborate with primary care physicians and other healthcare professionals to develop patient-centered care plans.
- Conduct thorough medical histories, reviews of medications, and observations of social environments to identify areas for improvement.
- Develop and implement treatment plans, including medication adjustments and referrals to specialists as needed.
- Provide education and support to patients and their families on disease management and healthy living strategies.
Requirements
- Master's degree in Nursing or a related field, or equivalent experience.
- Current licensure as a Nurse Practitioner or Physician Assistant, with prescriptive authority.
- Board certification in a relevant specialty.
- Minimum 1 year of experience in a similar role, with excellent communication and interpersonal skills.
- Ability to travel to patient locations, with valid driver's license and adequate automobile insurance.
