Key Responsibilities- Provide direct patient care, including assessment, planning, implementation, and evaluation of patient care plans.
- Collaborate with patients, families, and other healthcare professionals to develop and implement individualized care plans.
- Systematically collect and assess data related to patient health status to develop, execute, and evaluate the plan of care.
- Utilize evidence-based practice to drive clinical decisions and improve patient outcomes.
- Establish goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers.
- Consider factors related to safety, effectiveness, and fiscal accountability in planning and delivering care.
- Delegate and supervise care in accordance with the WI Nurse Practice Act.
- Participate in continuous quality improvement initiatives and assume responsibility for professional development.
- Clinical experience and/or interest in the care and management of patient populations specific to the area of employment.
- At least 1 year of relevant nursing experience preferred.
- Nursing licensure required. Bachelor's Degree in Nursing preferred.
- Current state of Wisconsin Registered Nurse License or Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact) required.
- American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA-approved equivalent required within 90 days of hire.
