Job Description

We are seeking a Senior Geriatric Home Care Nurse to provide high-quality, patient-focused care to veterans in their homes. In this role, you will be responsible for:

  • Developing and implementing individualized care plans to meet the unique needs of each patient.
  • Providing comprehensive geriatric care, including medication management, wound care, and palliative care.
  • Collaborating with interdisciplinary teams to ensure seamless care transitions and optimize patient outcomes.
  • Maintaining accurate records and reporting, ensuring compliance with regulatory requirements.
  • Participating in education and training programs to enhance clinical skills and knowledge.

 

Required Skills and Qualifications

To be successful in this role, you must possess:

  • A current, full, active, and unrestricted license as a registered nurse in any state or territory of the United States.
  • A minimum of 2 years of experience in geriatric care, preferably in a home-based setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • A commitment to delivering patient-centered care, with a focus on empathy, respect, and dignity.
