Clinical Risk Assessor

We are seeking a skilled Clinical Risk Assessor to join our team at Matrix Medical Network.

The successful candidate will conduct comprehensive health assessments, review medical history, and provide diagnosis and treatment recommendations. You will work collaboratively with healthcare professionals, case managers, and social workers to ensure seamless care coordination.

This is a PRN/Part Time opportunity, allowing for flexible scheduling and a competitive hourly rate of $95.00 per completed assessment.

• Conduct in-depth adult and geriatric assessments, including medical history, diagnosis, and treatment planning
• Collaborate with primary care physicians and other healthcare professionals to ensure comprehensive care
• Provide health education and guidance to patients and their families
• Travel to patient locations, including homes, skilled facilities, and via telehealth

To be considered for this role, you must have:

• A Master's Degree in a relevant field or equivalent experience
• Current licensure as a Nurse Practitioner or Physician Assistant, with prescriptive authority
• Board certification in a relevant specialty
• A minimum of 1 year of experience in a similar role
• Strong communication and interpersonal skills

You will also need to possess a valid driver's license and be able to travel frequently.

This PRN/Part Time opportunity offers a competitive hourly rate and mileage reimbursement plan.
