The Patient Care Assistant II provides necessary care to patients as designated by a written care plan developed by the RN Case Manager. This includes personal care and ADLs (Activities of Daily Living), such as bathing, oral hygiene, toileting, feeding, dressing, exercises, ambulation transfers, and medications that are ordinarily self-administered.

Required Skills and Qualifications

To be successful in this role, you will need:

  • A Certificate to practice as a Home Health Aide in the State of Connecticut or CNA with certification with the Connecticut State Nurse Aide Registry.
  • Ability to read, write, and speak English.
  • Ability to comprehend written and oral instructions in English.
  • Flexibility and ability to work well under pressure.
  • Excellent communication skills and ability to follow directions.
