We are seeking a highly skilled Clinical Pharmacist to join our team as a Home Infusion Specialist. As a key member of our Pharmacy team, you will be responsible for providing pharmaceutical care to patients in their homes, working closely with physicians, nurses, and other healthcare professionals to ensure the safe and effective use of medications.
Responsibilities- Evaluate and monitor drug therapy for home care patients, collaborating with healthcare professionals to optimize patient outcomes.
- Adhere to organization policies, state and federal laws, and accreditation standards governing pharmacy practice.
- Coordinate new referrals and admissions, facilitating seamless transitions for patients.
- Review and verify patient orders, documenting all clinical activities and communication.
- Provide consultation and answer inquiries from patients, families, and healthcare professionals regarding medications and their use.
- Maintain thorough documentation in patient records, processing and checking medications, supplies, and equipment prior to dispensing.
- Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
- Registered Pharmacist License in state(s) of practice.
- Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy.
- Experience with compounding infused drugs.
- Proven record of evaluating and monitoring drug therapies for home care patients.
- Good organizational and time-management skills.
- Participation and membership in professional societies and organizations is desirable.
- Excellent verbal and written communication skills.
