Key Responsibilities:- Provide clinical support for the initiation and monitoring of specialty medication therapy for self-administered medications.
- Develop and maintain clinical pharmacy pathways for the treatment of patients with complex conditions, such as Hepatitis C, Cancer, Cystic Fibrosis, HIV, Bariatrics, and Rheumatology.
- Collaborate with healthcare teams to ensure seamless patient care and optimal medication therapy outcomes.
- Stay up-to-date with the latest clinical guidelines and research to provide evidence-based care.
- Active and valid Indiana Pharmacist license.
- Four to five years of experience in a specialty, ambulatory, hospital-based, or closed-door long-term care pharmacy.
- Doctorate degree preferred.
