We're seeking a skilled Music Therapist to join our team. As a Music Therapist at Custom Home Health Inc, you'll play a vital role in providing tailored care to patients and their families in the comfort of their own homes.

• Conduct thorough assessments of music therapy needs for patients and their families
• Collaborate with interdisciplinary teams to prioritize interventions and referrals
• Meticulously document all care provided in adherence to hospice documentation standards
• Educate hospice staff and the community on the impactful role of Music Therapy
• Offer bereavement support to families when deemed appropriate
• Facilitate support groups as needed and ensure efficient resource utilization
• Fulfill additional duties as assigned with enthusiasm and dedication

• Bachelor's degree or equivalent in Music Therapy
• State licensure as a Board-Certified Music Therapist (MT-BC)
• A minimum of two years' experience in music therapy, with a preference for community/hospice experience
• Deep understanding of hospice philosophy, focusing on comfort and maintaining care in the patient's residence for as long as possible
