Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Accede Solutions Inc. as a Long Term Care Nurse. The successful candidate will be responsible for providing high-quality patient care and support to our residents in a fast-paced and dynamic environment.

Key Responsibilities
  • Observation and Reporting: Observe each resident on a continuing basis, alert to changes in mental and physical well-being, and notify the Supervisor of pertinent observations and discuss the need to notify physician.
  • Medication Administration: Accurately receive, transcribe, and carry out physician's orders, administer medications and treatments as ordered by the physician, and make proper documentation via a computerized system of MARs/TARs and electronic documentation.
  • Supervision and Leadership: Supervise Licensed Nursing Assistants (LNAs) in accordance with licensing requirements under the New Hampshire Board of Nursing, assign and measure nursing staff resident care consistent with abilities, job description, and scope of practice, and supervise the care given.
  • Collaboration and Communication: Accompany physician on resident rounds, address residents' medical needs, and facilitate communication during 60-day review, communicate with residents' families to resolve resident concerns and problems, and notify family members of new orders.
  • Administrative Tasks: Admit, discharge, and transfer residents, complete 24-hour report for assigned unit(s), check new orders, complete list of residents on antibiotics, precautions, etc., and maintain consistent communication with the Nurse Manager/Nursing Supervisor regarding resident issues or concerns with staff.
  • Quality Assurance: Follow State and Federal Regulations regarding the storage, usage, and documentation of controlled medications, including counting at the change of shift, and notify appropriate Supervisor if discrepancies are present.
  • Staff Development: Assist with the orientation of new nursing employees on assigned unit(s), communicate with DON, Nurse Manager, or SDC regarding continuing education needs of the nursing staff, and assist in the delivery of staff education and training as needed.
Requirements
  • 2+ years of current experience as a Licensed Practical Nurse (LPN)
  • Willingness to float to different units as needed
  • IV certification preferred
  • Previous travel experience and rural/small facility experience highly preferred
