Nurse Case Manager

The primary objective of this position is to oversee and manage all aspects of patient care, including supervision of Licensed Vocational Nurses (LVNs) and Home Health Aides (HHAs), case conferences, and discharge planning to identify the medical and social needs of patients. This role will promote patients' potential during their home health episode and collaborate with physicians, clinical teams, and patients to prepare for discharge.
Key Responsibilities:- Supervise and manage all aspects of patient care, including LVN and HHA supervision
- Coordinate the distribution of visits among all disciplines (Speech Therapist, Physical Therapist, Occupational Therapist, Social Worker, Home Health Aide) after the Start of Care, Recertification, or Resumption of Care is completed, and coordinate Plan of Care with the physician
- Initiate referrals to appropriate ancillary services, such as podiatry and community services
- Facilitate interdisciplinary communication to ensure that patients' needs and expectations for services provided are understood and to ensure optimization of service delivery and patient progress
- Lead weekly interdisciplinary case conferences for patient caseload and contribute to the development of care plans for patients
- Establish specific social action plans and realistic goals for each patient consistent with medically related social and emotional needs
- Coordinate all aspects of discharge planning with patients, clinical teams, and the agency
- Provide individual and group support for patients/residents and families at times of adjustment, crisis, or particular need
- Complete all necessary and supportive documentation, including assessments, evaluations, visit notes, and discharges, in accordance with state and federal regulations
- Ability to function as a team leader/role model
- Serve on various committees as requested by the Administrator
- Responsible for ensuring patient safety
- Registered Nurse, licensed per state requirements
- Skills in communication, documentation, assessment, and social work methods and techniques
- Ability to read, write, and speak the English language
