Key Responsibilities- Provide high-quality, patient-centered primary care services to individuals and families in a fast-paced, dynamic environment.
- Conduct thorough health assessments, develop and implement personalized care plans, and coordinate with other healthcare professionals as needed.
- Collaborate with our team of healthcare providers to ensure seamless care transitions and optimal patient outcomes.
- Stay up-to-date with the latest medical research, guidelines, and best practices to provide evidence-based care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellent patient care.
- Master's degree in Nursing (MSN) with a focus on Family Nurse Practitioner (FNP) or a related field.
- Minimum of 3 years' experience in a primary care or family medicine setting.
- Current licensure as a Registered Nurse (RN) and Nurse Practitioner (NP) in the state of practice.
- Certification as a Family Nurse Practitioner (FNP-C) or a related certification.
- Strong knowledge of workplace health and safety concepts, including OSHA regulations.
- Current certification in Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS).
- Ability to work effectively in a team environment and communicate with patients, families, and healthcare professionals.
- Strong computer skills and experience with electronic medical records (EMRs).
- Ability to adapt to changing priorities and deadlines in a fast-paced environment.
