Job Title: Advanced Care at Home RN Case Manager Northwest Suburbs IL

We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our Advanced Care at Home team in the Northwest Suburbs of IL. As a RN Case Manager, you will play a vital role in providing high-quality patient care and coordinating services to ensure the best possible outcomes for our patients.

Responsibilities:
  • Develop and maintain therapeutic relationships with patients and families throughout the healthcare continuum.
  • Regularly communicate and consult with the APC and MD Advanced Care at Home team regarding patient needs.
  • Assess patient's ongoing clinical status in person to determine healthcare needs or facilitate referrals to community resources.
  • Use the nursing process as a problem-solving model to recognize changes in clinical situations and evaluate and implement changes to the plan of care.
  • Plan, implement, coordinate, monitor, and evaluate all options and services to optimize patient health status, quality of life, and ability to remain in their home.
  • Educate patients and families regarding disease processes and trajectory and disease management.
  • Utilize teaching, assessment, and intervention skills to provide long-term or short-term case management to assigned patients.
  • Responsible for continuous review of all aspects of every patient's caseload to include appropriate utilization of services and make referrals to other team members as needed.
Requirements:
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA).
  • A valid driver's license issued by the Division of Motor Vehicles.
  • Registered Nurse license issued by the state in which the team member practices.
  • Bachelor's Degree in Nursing.
  • Typically requires 2 years of experience in home care nursing or 3-4 years of medical surgical experience.
Working Conditions:

The RN Case Manager will be required to travel to all areas within the ACAH Program service area in all extremes of weather on all types of roads. The job requires exposure to widely varying social and environmental conditions in patient's neighborhoods and homes. The RN Case Manager must be able to lift up to 35 pounds without assistance and use patient handling equipment when necessary.
