RN (Per Diem) - Hospice Care Specialist

We are seeking a compassionate and skilled RN Nurse Clinician to join our Hospice team. As a member of our interdisciplinary team, you will provide holistic care to terminally ill patients and their families, assessing their needs and developing individualized plans of care.
Key Responsibilities- Assess patient needs and develop comprehensive plans of care
- Collaborate with the IDG to anticipate changes in patient condition and promote patient and family choice
- Implement and revise plans of care as needed, ensuring continuity of care and enhancing the quality of life for dying patients
- Work collaboratively with the interdisciplinary team to provide holistic care
- Associate's or Bachelor's Degree in Nursing
- Washington Registered Nurse License
- 1 year of Nursing experience, with some pediatric nursing experience required for Pediatric Programs
