Job Title: Home Health Physical Therapist

Supplemental Health Care is seeking a skilled Physical Therapist to join our team in Overland Park, Kansas.

Job Summary:

We are looking for a dedicated Physical Therapist to work with our partnering home health care provider in Overland Park, Kansas. As a Physical Therapist with Supplemental Health Care, you will have the opportunity to work with a variety of patients and provide high-quality care in a dynamic and supportive environment.

Responsibilities:
  • Provide physical therapy services to patients in their homes
  • Develop and implement individualized treatment plans to meet the unique needs of each patient
  • Conduct thorough assessments and evaluations to identify patient needs and goals
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document patient progress and maintain accurate records
Requirements:
  • Graduate of an accredited PT program
  • Licensed as a Physical Therapist in Kansas
  • CPR Certified or ability to obtain
  • Previous experience with Computer Charting
  • 6 months of previous Home Health Care experience
  • Valid U.S. Driver's License and reliable transportation
