Key Responsibilities- Provide comprehensive nursing care to patients in their homes, including clinical assessment, evaluation, and management of patient conditions.
- Collaborate with our team of experts, including physicians, nurse practitioners, and other healthcare professionals, to develop and implement individualized care plans.
- Utilize advanced technology, including telehealth platforms and monitoring systems, to ensure seamless communication and care coordination.
- Manage patient medications, treatments, and therapies, as well as coordinate home health services and referrals as needed.
- Participate in ongoing education and training to stay current with best practices and advancements in home health care.
- Current RN license in Delaware or Compact State.
- BSN preferred or committed to acquiring BSN within 3 years of hire date.
- At least one year of med/surg experience as an RN required.
- BLS certification required.
