As a key member of our staff, you will play a vital role in supporting the health and well-being of our members by providing guidance and resources to help them achieve their wellness goals.

Key Responsibilities
  • Provide personalized support and guidance to members to encourage healthy behaviors and promote wellness services.
  • Develop and implement targeted outreach campaigns to engage members in wellness programs and health screenings.
  • Collaborate with internal departments to ensure seamless coordination of services and resources.
  • Maintain accurate and timely documentation of member interactions and program outcomes.
  • Provide exceptional customer service and communicate effectively with members, providers, and community partners.
Requirements
  • Bachelor's degree in a related field (e.g., health education, public health, social work) or equivalent experience.
  • At least 2 years of experience in a healthcare setting, preferably in health coaching, behavioral change, or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Valid California driver's license and proof of current automobile insurance.
Preferred Qualifications
  • Certification or licensure in a health-related field (e.g., CHES, CPH, RN).
  • Bilingual skills in a PHC threshold language.
  • Experience working with Medi-Cal and other special needs populations.
Work Environment

This is a standard cubicle workstation with a shared common area. The successful candidate will be required to sit in front of computer equipment and talk on the phone for a major portion of the workday. Face-to-face contact with members may be expected.
