As a Physical Therapist, you will be responsible for providing high-quality care to our patients, helping them to achieve their goals and improve their overall health and well-being.
Key Responsibilities- Provide physical therapy services to patients in their homes
- Develop and implement individualized treatment plans to meet the unique needs of each patient
- Conduct thorough assessments and evaluations to identify patient needs and goals
- Collaborate with other healthcare professionals to ensure comprehensive care
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care
- Current PT license as required by state
- CPR certification with American Heart Association or America Red Cross
- 1 year of experience in a healthcare facility
- Ability to meet physical demands of the job
- Positive attitude and dedication to quality patient care
- Strong communication skills and reliable transportation
