About the Role:
As a Nurse Care Coordinator, you will serve as a liaison between patients, families, and healthcare providers to ensure seamless care delivery. You will assess patients' needs, develop care plans, and coordinate referrals to specialists and community resources. Your strong communication and interpersonal skills will enable you to effectively advocate for patients and families, ensuring they receive the best possible care.
Key Responsibilities:
* Assess patients' needs and develop individualized care plans in collaboration with healthcare providers and patients/families.
* Coordinate referrals to specialists and community resources to ensure timely and effective care delivery.
* Communicate with patients, families, and healthcare providers to ensure clear understanding of treatment plans and coordination of care.

Requirements:
* Bachelor's degree in nursing or related field required.
* Current State of Connecticut Registered Nurse licensure required and maintained thereafter.
* Minimum 1 year of experience in nursing, preferably in a care coordination or case management role.
