POSITION SUMMARY:
The Inpatient Care Manager plays a pivotal role in collaborating with the clinical team and healthcare providers to ensure effective discharge planning and intervention. This position is dedicated to enhancing patient wellness, optimizing care outcomes, and ensuring efficient use of healthcare resources while minimizing payment denials for patients with intricate health requirements.
KEY RESPONSIBILITIES:
- Coordinate discharge planning efforts in conjunction with the clinical team.
- Implement interventions aimed at improving patient outcomes and resource utilization.
- Engage with patients to promote wellness and address complex health needs.
QUALIFICATIONS:
- A Bachelor's degree in a relevant field is preferred; a working knowledge of specialized practices is essential.
- A Registered Nurse (RN) license in Maryland is required; candidates from out of state must demonstrate intent to obtain MD licensure.
- Case Management certification is mandatory within 3-5 years of employment.
- 3-5 years of relevant experience in a healthcare setting is required.
