Job Summary

We are seeking a skilled Social Worker (MSW) to join our interdisciplinary team. As a Social Worker (MSW), you will provide psychosocial support to patients and their families, assessing their needs and developing personalized plans of care.

Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient and family needs
  • Develop and implement individualized plans of care to address identified needs
  • Provide emotional support and counseling to patients and their families
  • Collaborate with the interdisciplinary team to ensure comprehensive care
  • Document patient progress and outcomes
Requirements
  • Master's Degree in Social Work from a CSWE-accredited school
  • Licensure as required by the state in which the hospice is located
  • One year of experience as a Social Worker in a healthcare and/or hospice setting
