As an RN Case Manager/Administrator, you will play a pivotal role in overseeing and enhancing the quality of care provided to our clients. Your leadership will ensure the effective coordination of services and support for both clients and staff.
Key Responsibilities:
- Lead and supervise the delivery of professional and supportive services to clients and employees.
- Oversee and manage programs and staffing to ensure high-quality service delivery.
- Accountable for the management of Enhanced Community Living Facilities in Southern Ohio.
- Participate in the recruitment, onboarding, and ongoing training of staff members.
- Monitor and manage financial performance to align with organizational goals.
- Conduct client assessments and direct care planning in home health scenarios.
- Supervise a team of Registered Nurses (RN), Licensed Practical Nurses (LPN), Home Health Aides (HHA), and Personal Care Aides.
- Document and communicate patient status and any changes to the care team effectively.
- Ensure personal safety and maintain a safe environment for clients and colleagues while performing job duties.
- Willingness to travel to surrounding areas as needed.
- Minimum of one (1) year of supervisory nursing experience is required, with a preference for home care experience.
Qualifications:
- Nursing Experience: 1 year of supervisory experience (Required)
