Position Overview:

The Hospice Triage Registered Nurse plays a crucial role in assessing and addressing the needs of patients and their families through incoming and outgoing communications.

Work Schedule:
Full-time, Monday to Friday, from 8:30 AM to 4:30 PM, with additional evening and weekend hours as required.

Key Responsibilities:
- Evaluate and determine the needs of patients and families based on phone interactions.
- Triage patients to the appropriate level of care with urgency.
- Provide assessments, scheduling, clinical guidance, and other essential services to patients.
- Uphold safety, quality, and patient experience aligned with the mission of Catholic Health.
- Understand patient needs and offer recommendations, education, or care advice to facilitate appropriate scheduling.
- Assign nursing visits as necessary.

Qualifications:
- Minimum of 3 years of relevant experience.
- Current and unrestricted RN License in New York State.

Salary Information:
The estimated salary for this position is USD $55.00 per hour. This range is a good faith estimate and actual compensation may vary based on qualifications, skills, competencies, and experience.
