- Provide Education and Support: Educate patients and families on clinical care, addressing their questions and concerns.
- Facilitate Communication: Foster open communication among team members to ensure progress in meeting expected care and service outcomes.
- Assist Team Members: Support team members in building strong relationships with clinic staff, clinicians, and interdisciplinary team members involved in patient care.
- Serve as a Resource: Provide guidance and support to team members to enhance successful implementation of care plans.
- Current RN License: Possess a current RN license in the state of Minnesota.
- Education: Graduate of an accredited RN program, Associate degree required, BSN preferred.
- Experience: Previous RN experience in a clinic system or care coordination preferred.
- CPR Certification: Hold current CPR certification.
