At Abraxas Youth and Family Services, we are seeking a skilled Transition Specialist to join our team. As a Transition Specialist, you will play a critical role in supporting students in their transition from our program to their next steps in life.
Key Responsibilities:
- Meet with new students to coordinate their schedule and intake meeting with school staff.
- Create a profile of each new student to disseminate information to staff regarding student strengths, weaknesses, and IEP information.
- Research and coordinate field trips to college and career opportunities for students.
- Work with students to complete the FAFSA for future school financing.
- Facilitate potential housing needs and provide linkage to home schools and community agencies.
- Assist students in developing a Student Success Plan (SSP) to chart their steps and measurements for a successful return to their home community.
Requirements:
- Bachelor's degree in a related field and one year of related experience, or a Master's degree in a related field and six months of related experience.
- Excellent grammar, communication, and organization skills.
- At least 21 years of age.
- Criminal clearances, pre-employment physical exam, valid driver's license, and successful completion of a pre-employment drug screening.
