Job Summary
We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Acute Rehabilitation. As an LPN, you will provide high-quality patient care under the supervision of a Registered Nurse (RN) or physician. Your expertise will be essential in assessing patients, administering medications, and educating patients and families about disease treatment plans.
Responsibilities
- Provide basic bedside care to long-term care patients under the direction of a Registered Nurse (RN) or physician.
- Contribute to the assessment of patients by collecting data for analysis by a Registered Nurse (RN).
- Administer medications and observe patients for adverse reactions to medications or treatments.
- Perform routine laboratory tests and simple therapeutic services.
- Educate patients/families about disease treatment plans, including self-care post-discharge, holistic health needs, available resources, and follow-up care.
- Assure resident safety by identifying safety hazards and emergency situations and initiating corrective action as appropriate.
Requirements
- Licensure/Certification/Registration: BLS Provider obtained within 1 month of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Licensed Practical/Vocational Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
- Education: High School diploma equivalency or 1 year of applicable cumulative job-specific experience required.
Preferred Qualifications
- Previous experience in long-term care settings preferred.
- Excellent communication skills with peers and patients.
- Ability to prioritize and flex throughout the fast-paced shift.
- Able to take an assignment, pass medications, document data collection in an accurate and timely manner.
