About the Role: We are seeking a skilled Nurse Practitioner to join our team at Abode Care Partners. Key Responsibilities:
- Deliver high-quality medical care to patients, including examining and treating chronic and urgent illnesses, minor injuries, accidents, and other injuries or illnesses.
- Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care.
- Ensure patient medical records are kept up to date.
- Develop and implement individualized care plans that meet patients' unique needs.
Requirements: To be considered for this role, you must have an active state Nurse Practitioner license, current National Board Certification, and DEA licensure. A Master's degree in Nursing (MSN) is also required, along with five or more years of nursing experience and at least two years of nurse practitioner experience. Strong interpersonal communication skills, time management abilities, and proficiency in medical instruments and equipment are essential.
