Job Overview

HAMILTON-MADISON HOUSE is a non-profit organization dedicated to providing comprehensive social services and healthcare to the community. We are seeking a highly qualified Behavioral Health Counselor to join our team.

Responsibilities
  • Provide assessment and treatment services, including individual, family, and group counseling, to patients with substance abuse and mental health issues.
  • Conduct comprehensive psychosocial assessments for patients and apply various counseling therapy approaches, such as MI and CBT.
  • Maintain accurate clinical records, including comprehensive evaluations, treatment plans, progress notes, and reports.
  • Provide information about addiction and available services, making referrals when necessary.
  • Coordinate counseling efforts with mental health professionals, other healthcare providers, and justice system entities.
  • Participate in outreach activities and maintain medical records documentation according to HMH and OASAS policies.
  • Meet productivity and quality assurance goals set by the agency.
