- Serve as an advocate for clients.
- Evaluate and assess the health status of assigned clients.
- Collaborate with clients, their families, and the healthcare team to identify needs and set achievable care goals.
- Adjust and update care plans according to the evolving needs of clients.
- Administer medications, IV therapies, and provide education and counseling to clients.
- Assess the home environment to ensure safety and comfort.
- Coordinate additional services for clients as necessary.
- Engage in team meetings and maintain accurate client records.
- Promote safety for both clients and colleagues.
- Participate in ongoing professional development and mentor other nursing staff.
- Maintain accountability for all delegated nursing tasks.
- Carry out other assigned responsibilities as needed.
