About the Role

The successful candidate will be responsible for developing and implementing individualized nutrition plans of care, collaborating with the interdisciplinary team to ensure seamless care delivery, and providing education and support to residents, families, and caregivers.

Responsibilities
  • Develop and implement nutrition care plans that meet the unique needs of each resident
  • Collaborate with the interdisciplinary team to ensure comprehensive care delivery
  • Provide education and support to residents, families, and caregivers on nutrition-related topics
  • Monitor and evaluate the effectiveness of nutrition interventions
  • Ensure accurate and timely documentation of nutrition assessments and interventions
Requirements
  • Registered Dietitian (RD) or licensed dietitian (LD) as required by the state of employment
  • Previous experience in a healthcare setting or as a consulting dietitian preferred
  • Experience working with geriatric populations is a plus
