">The Role
">As a Nurse Practitioner with Abode Care Partners, you will provide evidence-based medical care to patients, working collaboratively with healthcare professionals to develop comprehensive treatment plans.
You will be responsible for diagnosing and treating illnesses, managing patient records, and ensuring seamless continuity of care.

">Responsibilities
">- Evidence-Based Medical Care: Plan, direct, and evaluate patient medical needs to ensure the highest standard of care.
- Chronic and Urgent Care: Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other conditions as needed.
- Laboratory and Diagnostic Testing: Order and interpret laboratory and diagnostic testing to inform treatment decisions.
- Collaboration and Communication: Work with caregivers and healthcare professionals to maintain a comprehensive plan of care and facilitate interdisciplinary collaboration.
- Patient Referrals: Provide referrals to other healthcare practitioners when necessary to ensure optimal patient outcomes.
- Medical Record Management: Ensure accurate and up-to-date maintenance of patient medical records.
Requirements
">- Licensure and Certification: Active state Nurse Practitioner license, current National Board Certification, and active DEA licensure.
- Education: Master's degree in Nursing (MSN).
- Experience: Five or more years of nursing experience, with at least two years of nurse practitioner experience preferred.
- Skills: Strong interpersonal communication skills, excellent time management skills, proficiency in medical instruments and equipment, and knowledge of HIPAA and OSHA regulations.
- Technology: Proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
- Physical Requirements: Ability to engage in moderate physical activity, including handling objects up to 25 lbs., standing or walking for more than four hours per day.
