Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Alden Estates of Evanston. As an LPN, you will play a vital role in providing high-quality patient care and supporting our residents in achieving their optimal health and well-being.

Job Summary

The LPN will be responsible for providing direct nursing care to our residents, including administering medications, performing routine medical procedures, and monitoring patient conditions. You will also be responsible for supervising and instructing nursing assistants, ensuring that they are following established policies and procedures.

Responsibilities
  • Provide direct nursing care to residents, including administering medications, performing routine medical procedures, and monitoring patient conditions.
  • Supervise and instruct nursing assistants, ensuring that they are following established policies and procedures.
  • Develop and implement individualized care plans for residents, taking into account their unique needs and preferences.
  • Collaborate with other healthcare professionals, including physicians and therapists, to ensure that residents receive comprehensive and coordinated care.
  • Document resident care and progress in a timely and accurate manner, using electronic medical records and other tools as needed.
  • Participate in ongoing education and training to stay up-to-date on the latest nursing practices and technologies.
  • Contribute to a positive and supportive work environment, promoting a culture of respect, empathy, and compassion among colleagues and residents.
Requirements
  • Current LPN licensure in the State of Illinois and/or Wisconsin.
  • Current CPR certification or willingness to obtain.
  • Ability to read, write, and speak English fluently.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
  • Ability to lift, push, pull, and move equipment and supplies as needed.
  • Flexibility and adaptability, with the ability to work varied shifts and respond to changing resident needs.
