Job Title: Patient Onboarding Coordinator

Location: Williamsburg (On-site)

Estimated Salary: $62,500 per year, including a base salary of $50,000 and potential performance-based commissions of up to $12,500.

Job Description:

We are seeking a detail-oriented and customer-focused Patient Onboarding Coordinator to join our team at Anchor Health Homecare Services, Inc. As an Enrollment Specialist, you will play a crucial role in ensuring the smooth onboarding and enrollment of new patients and caregivers. This includes initiating patient intake, verifying Medicaid eligibility, assisting with documentation, negotiating caregiver rates, and liaising with internal teams such as HR and Marketing.

Responsibilities:

  • Enroll Patients and Aides: Handle intro calls, provide a sales pitch, verify Medicaid, assist with enrollment, and follow up on onboarding.
  • External & Internal Communication: Act as the liaison between marketers, patients, caregivers, and other internal teams.
  • Schedule Assessments: Coordinate patient assessments with appropriate teams.
  • Document Collection: Ensure all required documents are collected and submitted.
  • Process Authorizations: Follow up with coordination and staffing teams to ensure authorizations are processed in a timely manner.

Success Measurements:

  • Day 1: Arrive on time, ready with onboarding materials, and take notes during training.
  • 30 Days: Manage a caseload of 40-75 cases and obtain your first authorization.
  • 60 Days: Manage 80-120 cases without repeating mistakes from the first 4 assessments.
  • 90 Days: Process 15 authorizations monthly with a caseload of 130+.
  • 6 Months: Fully proficient with all systems and processes, managing a full caseload of 175-250 and supporting new representatives.
  • 1 Year: Achieve an average of 25-30 authorizations per month, maintaining a full caseload, supporting other representatives on the team.

Requirements:

  • Experience: Prior experience in a customer service or sales-focused role, ideally in healthcare or homecare settings.
  • Skills:
  1. Proficient in Microsoft Office Suite (Excel, Word).
  2. Strong organizational skills and multitasking ability.
  3. Personable, with excellent communication and negotiation skills.
  4. Ability to work independently.
