Job Summary

We are seeking a highly skilled Family Practice/Primary Care Nurse Practitioner to join our team at University Medical Partners | Stanford Medicine. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our physicians and staff to deliver exceptional patient outcomes.

Key Responsibilities:
  • Provide primary and preventive care to patients of all ages, including health promotion, disease prevention, and management of acute and chronic illnesses.
  • Conduct thorough health assessments, develop and implement individualized care plans, and coordinate care with other healthcare providers as needed.
  • Order and interpret diagnostic tests, prescribe medications, and provide education to patients and their families on healthy lifestyle choices and disease management.
  • Collaborate with our multidisciplinary team to ensure seamless care transitions and optimal patient outcomes.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices in family medicine and primary care.
Requirements:
  • Master's degree in Nursing or related field.
  • Current licensure as a Nurse Practitioner in the state of California.
  • Board certification in Family Medicine or Primary Care.
  • Minimum 2 years of experience as a Family Practice/Primary Care Nurse Practitioner.
  • Excellent communication and interpersonal skills.
