Job Summary:
Care Team Solutions is seeking a skilled Registered Nurse (RN) to join our team in a Long Term Care setting. As a RN Nights, you will be responsible for providing high-quality patient care and support to our residents.
Responsibilities:
- Provide direct patient care to residents in a Long Term Care setting
- Develop and implement individualized care plans to meet the unique needs of each resident
- Collaborate with the interdisciplinary team to ensure seamless care delivery
- Monitor and report any changes in resident condition to the healthcare team
- Participate in quality improvement initiatives to enhance patient outcomes
Requirements:
- 2+ years of recent Long Term Care experience
- Current RN licensure in the state of practice
- Ability to work 12 hour overnight shifts
- Strong communication and interpersonal skills
