About the Role

We are seeking a compassionate and skilled Registered Nurse to join our Hospice team as a RN Case Manager. As a RN Case Manager, you will play a vital role in providing high-quality patient care and support to patients and their families.

Key Responsibilities
  • Provide direct patient care services to hospice patients and supportive care to the patient's family.
  • Act as primary liaison between physician, patient/family and hospice team.
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
  • Manage and collaborate with an interdisciplinary group consisting of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA) and support staff to provide comfort, peace, and dignity.
  • Maintain all assigned patient clinical records.
  • Participate in company quality assessment and performance improvement programs.
About You

We are looking for a RN Case Manager who is passionate about delivering exceptional patient care and has a strong commitment to the values of compassion, empathy, and respect. If you are a motivated and dedicated professional with a strong nursing background, we encourage you to apply.

Requirements
  • RN license in the state of requested employment (or eligibility to obtain).
  • 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
  • Additional preferred experience: hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry or travel nursing.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
