We invite skilled Registered Nurses to become part of our team as Home Health Case Managers. In this critical role, you will deliver compassionate and comprehensive care to patients in need.
Your responsibilities will include assessing patients' conditions, developing tailored care plans, and collaborating with healthcare professionals to ensure optimal outcomes. You will also have the opportunity to educate patients and their families on disease management, medication adherence, and healthy lifestyle choices.
This position offers a rewarding salary range of $75,000 - $80,000 per year, along with the chance to work autonomously, enjoy 1:1 nurse-to-patient ratios, and take advantage of tuition discounts through Rasmussen University.
Key Requirements:
- A current RN license in Kansas
- Clinical expertise in nursing specialty
