We're seeking a skilled Nurse Practitioner to join our team, providing primary, urgent, and acute medical care to patients in their homes. As a key member of our care community, you'll work closely with our physicians, social workers, and other clinicians to deliver high-quality care and support to our patients.
Responsibilities will include:
- Providing primary care to patients in their homes
- Conducting initial patient screenings and physical examinations
- Creating assessment and care plans, addressing urgent care needs as needed
- Collaborating with our community team to complete care assessments
- Ordering and interpreting diagnostic tests, determining the need for follow-up appointments and further treatment options
- Prescribing and managing medications and treatments as appropriate
- Updating patient records and ensuring accuracy at each patient appointment
