Nurse Practitioner

Job Summary:
We are seeking a skilled and compassionate Registered Nurse to join our team as an On-Call Registered Nurse. As a key member of our hospice care team, you will provide high-quality patient care and support to individuals and families in their homes and communities.
Responsibilities:
- Provide comprehensive assessments and develop individualized care plans to meet the unique needs of patients and families.
- Deliver high-quality patient care, including medication management, wound care, and symptom management.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Respond to after-hours calls and provide emergency care as needed.
- Document patient information and progress in a timely and accurate manner.
- Participate in ongoing education and training to enhance clinical skills and knowledge.
Requirements:
- Graduate of an accredited school of nursing with a current RN license in the state.
- One to two years of recent acute care experience in an institutional setting and one to two years of recent experience in home care.
- CPR certification and ability to lift, position, and transfer patients.
- Excellent observation, verbal, and written communication skills, problem-solving skills, and basic math skills.
