We are seeking a highly skilled Physician in Geriatric Care to join our team at Fallon Community Health Plan. As a Geriatric Care Physician, you will play a vital role in providing medical oversight to patients in our facilities, coordinating with our Primary Care Physicians, and participating in daily Interdisciplinary Team (IDT) meetings.
Responsibilities- Provide medical oversight to patients in our facilities, ensuring high-quality care and adherence to regulatory protocols.
- Coordinate with our Primary Care Physicians to ensure seamless care transitions and effective communication.
- Participate in daily IDT meetings to discuss patient care plans, address concerns, and make informed decisions.
- Develop and implement care plans that cater to the unique needs of our patients, focusing on maximizing their dignity and respect.
- Collaborate with our interdisciplinary team to provide comprehensive care, including medical, social, and rehabilitative services.
- Medical degree, with a strong background in Geriatrics or a related field.
- Board certification in Internal Medicine, Geriatrics, or Family Practice is preferred.
- At least two years of experience working with frail or elderly populations, with a focus on geriatric care.
- Previous management experience is desirable.
