INTEGRIS Health, the largest not-for-profit health system in Oklahoma, offers an exciting opportunity for a Registered Nurse Case Coordinator. In this role, you will be part of our dedicated Case Management team, working full-time during the day to provide outstanding care to our patients.
Key Responsibilities:
- Conduct comprehensive assessments of patients' clinical, psychological, and financial needs using all available resources.
- Facilitate timely transfers to appropriate levels of care based on clinical requirements and utilization criteria.
- Develop, implement, and revise discharge plans, including referrals to healthcare and community organizations.
- Communicate discharge care plans and any changes to patients, families, and healthcare professionals.
- Assist physicians and hospital staff in resource utilization and timely discharge planning.
- Coordinate services across hospital departments to ensure efficient patient discharge.
- Perform concurrent reviews of patient records upon admission and as needed based on clinical conditions.
- Apply utilization criteria to determine the appropriate use of resources.
- Notify relevant contacts of any utilization issues that may impact patient care or reimbursement.
- Act as a resource for physicians regarding discharge planning and documentation.
- Maintain knowledge of CMS regulations, Medicare/Medicaid, and managed care guidelines.
- Provide education to patients and families about healthcare regulations and resources.
- Identify opportunities to reduce costs while maintaining quality patient care.
- Participate in data collection for utilization and quality improvement initiatives.
Qualifications:
- Current licensure as a Registered Nurse (RN) in the State of Oklahoma or a multistate license from a Nurse Licensure Compact (eNLC) member state.
- Five years of experience in various clinical settings is required.
- Experience with managed care and payer/provider requirements is preferred.
- BSN is preferred, along with excellent communication and collaboration skills.
- Computer proficiency is required, with a preference for Windows experience.
- Current Case Management Certification is preferred, to be obtained within one year of hire.
- Possession of a valid Driver's License issued by the state of Oklahoma.
