Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at VNA & Hospice of the Southwest Region. As a hospice LPN, you will provide individualized goal-directed nursing care to patients in their homes, under the supervision of an RN or Clinical Manager.
Key Responsibilities- Provide quality care to assigned patients, ensuring their needs are met and their comfort is maintained.
- Carry out the plan of care for each patient, ensuring that the patient/caregivers are included in the care plan process.
- Ensure documentation is completed, communicating via a note the activities and needed actions of each visit to the RN.
- Implement medical plan of care, nursing interventions, and procedures as necessary to ensure the safe, effective, and quality care for patients.
- Evaluate the effectiveness of the implemented plan of care, making recommendations to the case manager as needed.
- Licensed Practical Nurse (LPN) license
- Prior clinical acute care nursing experience required; prior home care experience preferred
- Valid Driver's License, Vehicle, and Insurance; ability to pass motor vehicle record check, child abuse, criminal record check, and adult abuse check
