Clinical Pharmacist Position

We are seeking a highly skilled Clinical Pharmacist to join our team at Memorial Healthcare System. As a Clinical Pharmacist, you will play a critical role in providing exceptional patient care and contributing to the success of our healthcare system.

The Clinical Pharmacist will be responsible for providing clinical pharmacy services, including medication therapy management, patient education, and medication use evaluation. You will work closely with our healthcare team to ensure the highest quality of care for our patients.

• Provide clinical pharmacy services, including medication therapy management and patient education
• Conduct medication use evaluations and make recommendations for improvement
• Collaborate with healthcare team members to ensure high-quality patient care
• Participate in quality improvement initiatives and contribute to the development of policies and procedures

To be successful in this role, you will need:

• PGY2 residency plus one (1) year of clinical experience or PGY1 residency/fellowship plus three (3) years as a clinical pharmacist in a hospital pharmacy setting
• Active board certification in an applicable field plus two (2) years as a clinical specialist
• Residency trained or board certified in an applicable field
