We are seeking a skilled RN Clinical Liaison to join our team at Curo Health Services. As a RN Clinical Liaison, you will play a critical role in ensuring the effective intake and referral processes for patients, families, and referral sources.
Key Responsibilities- Ensure timely and accurate completion of admissions processes for referred patients
- Provide education and information regarding hospice services to referral sources and patients
- Act as a patient advocate in evaluating and developing plans of care
- Collaborate with healthcare professionals to promote hospice services and provide end-of-life solutions
- Develop and maintain relationships with referral sources, including SNFs, ALFs, hospitals, and physicians
- RN license in the state(s) of employment
- Minimum of 2 years of healthcare experience in hospice, home health, clinical liaison, or related settings
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
